The Champlain Local Health Integration Network’s (LHIN) mandate is to ensure health services are well-organized, appropriately funded, and meet the health needs of the 1.3 million residents who call this region home. We work with and fund roughly 120 health service providers that offer about 240 health programs in hospitals, community support services, mental health and addiction services, community health centres, and long-term care homes. The Champlain LHIN also delivers home and community care services.

We are recruiting for an Occupational Therapist. This is a regular, full-time position.

The Occupational Therapist completes assessments, develops treatment plans, organizes, provides and directs occupational therapy treatment and educational programs for Community Care Access Centre clients, their families and caregivers in accordance with the standards of the College of Occupational Therapists of Ontario.

Primary Responsibilities:

  • Completes assessments of clients in a variety of treatment settings (private homes, schools, seniors' residence, long-term care facilities, group homes) and performs an analysis based on objective, subjective and functional findings.
  • Establishes realistic, measurable and time limited treatment goals and develops treatment plan.
  • Ensures that occupational therapy treatments are initiated with the informed consent of the client or substitute decision maker in accordance with the Health Care Consent Act.
  • Provides direct treatment and/or support and teaching to informal caregivers, school personnel and other health care professionals involved.
  • Participates in case conferences when required and consults with other members of the health care team in fostering a holistic approach.
  • Completes documentation and maintains appropriate client's records in accordance with the standards of the College of Occupational Therapists of Ontario and the Community Care Access Centre policies and procedures.
  • Completes assessments and recommendations for adaptation aids, assistive devices and equipment necessary to achieve treatment goals respecting where appropriate the requirements of the Ontario Assistive Devices Program.

Education and Experience:

  • Registration with the College of Occupational Therapists of Ontario
  • Minimum of two (2) years of recent clinical experience
  • Community experience working with children and the elderly is considered an asset
  • Valid CPR certificate


  • Medical conditions and cognitive and psychiatric disorders
  • Client-centered, multi-disciplinary team approach to client care

Skills and Abilities:

  • Ability to establish rapport and maintain empathetic and therapeutic relationships
  • Ability to function effectively as a member of a multi-disciplinary team
  • Ability to work independently while exercising sound professional judgment
  • Proficient in the use of computerized processes including the use of mobile (lap top) technology
  • Ability to effectively participate in care planning to meet clients’ physical, psychosocial, cultural and emotional needs
  • Demonstrated time management and organizational skills
  • Effective decision-making and problem-solving in crisis situations
  • Ability to negotiate and implement an effective goal-oriented, client-centered care plan
  • Team player with ability to accept and adapt to change
  • Ability to maintain a positive attitude in stressful situations
  • Ability to organize daily workload in the presence of frequent interruptions
  • A valid Ontario driver’s licence and access to a vehicle are required.


  • Valid N95 Mask Fit Test results or documentation of a medical exception within the past two years
  • Vulnerable sector check completed within the last six months
Additional Information

Wages are in accordance with the collective agreement between Champlain LHIN and OPSEU and starting salaries range from $39.22-$41.89 per hour.

  • Shift: Monday to Friday; 8:30am-4:30pm
  • Location: Pembroke
